We work to support children and young people with complex disabilities

Children’s Disability Network Teams (CDNTs) deliver services through an interdisciplinary model which supports families, schools and other services in meeting the needs of the child.

This website provides information on the 11 CDNTs in the Community Health Organisation 7 (CHO7) Area which covers Dublin South, Kildare, and West Wicklow.

How to refer a child to one of our teams

Referrals

Children can be referred by their parents or guardian, health and social care professionals, GPs, hospitals, education professionals, Assessment Officers, or Case Managers. A child’s home address will determine the CDNT they are referred to.

Our Teams

All CDNTs include health and social care professionals (HSCPs) who specialise in different areas of child development. Their aim is to support your child to thrive, grow, and learn.
