How to refer a child or young person to one of our teams

Referrals

A child’s or young person's home address will decide the CDNT they are referred to. Children can be referred by their parents or guardian, health and social care professionals, GPs, hospitals, education professionals, Assessment Officers, or Case Managers.

Our Teams

All CDNTs include health and social care professionals (HSCPs) who specialise in different areas of child development. Their aim is to support your child or young person to thrive, grow, and learn.
